    Real-World Study on Use of Maixuekang Capsules Combined with Western Medicines in Treating Coronary Heart Disease

    Objective To analyze the combined use of Maixuekang capsules with western medicines in patients with coronary heart disease in the real world in order to provide data for the treatment of coronary heart disease with integrated traditional Chinese and western medicines. Methods The medical data on coronary heart disease inpatients who took Maixuekang capsules was retrieved from the information system database of 22 tertiary hospitals across the country from 2006 to 2016, and the association between Maixuekang capsules and western medicines was analyzed. Results Maixuekang capsules were most commonly used in combination with such western medicines as aspirin, isosorbide dinitrate, alprostadil, nifedipine and metoprolol in the treatment of coronary heart disease. The pharmacological types of western medicines were anticoagulants, vascular active drugs, calcium ion antagonists and antihypertensive drugs. Conclusion Maixuekang capsules are often used in combination with conventional western medicines in the treatment of coronary heart disease in the real world, and the medication regimens comply with clinical guidelines.

    Characteristics of Clinical Combination with Yinzhihuang Granules in the Real World

    Objective To analyze the way Yinzhihuang granules for injection are used in the real world in order to provide reference for rational drug use. Methods Based on the real world electronic medical record system (hospital information system, HIS), the data on diagnosis and treatment of patients treated with Yinzhihuang granules was retrieved while the basic information of patients and diagnosis was analyzed descriptively. The combined use of related drugs was mined using association rules of the Apriori algorithm to explore how Yinzhihuang granules were used in clinical practice. Results A total of 5310 patients treated with Yinzhihuang granules for injection in 21 tertiary hospitals between June 2010 and June 2016 were included in the study. Male patients outnumbered female ones. Most of them were aged 45 to 65. The common diseases treated with Yinzhihuang injection were neonatal hyperbilirubinemia, pulmonary tuberculosis and viral hepatitis B. The common drug regimens were Yinzhihuang granules plus Kuhuang injection / Chaihu injection plus glycyrrhizic acid and pyrazinamide combined with rifampicin and ethambutanol. Conclusion The indications and dosage related to Yinzhihuang granules are basically in line with the relevant guidelines. Two common clinical treatment schemes have been found, which are of referential value in promoting the rational use of Yinzhihuang granules.

    Analysis of Drug Characteristics of Mailuoshutong Pill Treating Patients with Orthopedic and Surgical Disease in Real World

    Objective To find out about the clinical characteristics of combined use of Mailuoshutong pills in the treatment of orthopedic and surgical diseases. Methods Information about 3173 patients with orthopedic and surgical diseases who used Mailuoshutong pills at least once in the information system of 14 large tertiary hospitals in China between 2006 and 2016 was retrieved. The clinical characteristics of drug combination were described. Results The average age of these patients was (42.74±16.33) years old, and the ratio of males to females was 2.54∶1. The majority of patients were 18-45 years old (46.14%). Among the combined drugs, Honghua injection (9.77%) was most likely to be used with Mailuoshutong pills. Phenobarbital (6.4%) was the most common drug used in combination with one type of western medicine. Association rules showed the combination of one type of traditional Chinese medicine and one type of western medicine was usually between Xianling Gubao capsules and cefazolin (9.39%). The combination of one type of traditional Chinese medicine and two types of western medicines was usually between Xianling Gubao capsules and cefazolin and phenobarbital (8.54%). The combination of one type of traditional Chinese medicine and three types of western medicines normally involved Xianling Gubao capsules, cefazolin, phenobarbital and inosine (4.92%). Conclusion Mailuoshutong pills are often combined with such traditional Chinese medicines that can invigorate blood circulation and remove blood stasis, tonify the kidney and strengthen bones, invigorate the spleen and facilitate defecation. These pills are also often combined with western medicines that are sedative, painkilling, antibacterial, anti-inflammatory, acid suppressive and stomach-protecting. This study has provided data for clinical drug combination against orthopedic and surgical diseases. Further verification is needed to carry out clinical studies at a higher level of evidence.

    Objective To explore the demographic characteristics and patterns of medication among patients using breviscapine injection in the real world. Methods The medical data on patients treated with breviscapine injection was retrieved from the information system of 34 tertiary hospitals from January 1, 2002 to December 31, 2015 in China. After normalization, a model was established using the Apriori algorithm, and association rule analysis was carried out with Clementine12.0 software. Results Most of the 48 710 patients were between 45 and 64 years old (34.66%), and male patients outnumbered female ones. Departments of Orthopedics (19.14%) and Cardiology(14.83%) were the biggest users of the drug. The top three diagnoses by Western medicine were essential hypertension (19.33%),coronary heart disease (12.39%),and cerebral infarction (11.25%),while the dominating syndromes diagnosed by TCM were liver and kidney Yin deficiency syndrome (17.29%),followed by qi stagnation and blood stasis syndrome (16.60%),and phlegm and blood stasis syndrome (8.78%). The route of administration was mostly intravenous drip, and the single dose was mostly 20-50mg (51.47%).The most frequent combination with western medicines involved acetylsalicylic acid (4.59%) and heparin (3.04%) , compared with compound salvia miltiorrhiza dropping pills (2.12%) and Simo decoction (1.84%) from TCM. Conclusion The results showed that the patients with breviscapine injection were mainly middle-aged and elderly patients, and the indications were basically in line with the instructions, but there were still off-label drug use (orthopedic disease treatment), and the proportion of off-label dose in actual clinical use was relatively high, it is necessary to strengthen the dosage specification and medication supervision. The clinical application should still refer to the instructions. Combined drug use is consistent with the guidelines and has certain rules to follow, which can provide clues for clinical medication.

    Tumors are difficult to cure and pose a serious threat to human lives. Current approaches to tumors include surgery, radiotherapy, chemotherapy, drugs targeting and immunotherapy. In recent years, with more research on tumor immunology and progress in such disciplines as immunology and molecular biology, immunological treatment of tumors has proved to be of good clinical value with advantages in clinical practice. Novel anti-tumor drugs for immunological treatment keep emerging and promise a good prospect of development. Based on the immunity treatment of tumor, this article discussed the basic categories and research progress regarding the new anti-tumor immunity treatment medicine. At the same time, the problems of this medicine and the challenges that China faces in this field were summarized to provide reference and help for the research of new anti-tumor immunity medicine, clinical reasonable medication and anti-tumor treatment.

    Usage of New Anti-cancer Drugs in Nanjing from 2018 to 2020

    Objective To analyze the data on procurement of new anti-cancer drugs in Nanjing between 2018 and 2020 and find out about the recent usage of such drugs and developments in order to provide reference for clinical rational use of new anti-cancer drugs and national medical insurance admittance. Methods Based on the procurement data by Jiangsu Institute of Medical Information in Nanjing from 2018 to 2020, the DDDs, consumption amount, defined daily dose cost (DDDc) and B/A value were retrospectively analyzed. Results The consumption amount and constituent ratio of new anti-cancer drugs continued to rise. DDDs increased rapidly, the DDDs of PD1 drugs increased significantly, but DDDc of most of these drugs continued downward. The DDDc of most monoclonal antibody drugs was high. The B/A values of most drugs were elevated while drug synchronization improved. Conclusion The usage of new anti-cancer drugs is on the increase, and these drugs are entering the medical insurance directory one after another. The availability and affordability of new anti-cancer drugs have been significantly improved. These drugs are being used more rationally.

    Market Competition Trends of Innovative Anti-tumor Drugs in Nanjing Based on Terminal Data

    Objective To analyze and evaluate the current market competition and developments of new anti-tumor drugs in Nanjing, and recommend countermeasures to enhance the competitiveness of China's pharmaceutical industry. Methods Based on the statistics of the consumption of innovative anti-tumor drugs used in 35 secondary and tertiary hospitals in Nanjing between 2016 and 2019, the types, varieties, sales amount, market share of innovative anti-tumor drugs and the sales distribution of medicines produced by domestic or overseas manufacturers were analyzed. Results The sales amount of new anti-tumor drugs increased at an annual average rate of 41.27%. Cytotoxic drugs held almost 60% of the market share of innovative anti-tumor drugs. Among molecular targeted drugs, monoclonal antibodies and multi-target tyrosine kinase inhibitors were growing rapidly. New anti-tumor drugs accounted for about 20% of the total sales amount, and home-made new anti-tumor drugs accounted for about 60%. Conclusion The sales amount of innovative anti-tumor drugs in the terminal market in Nanjing is trending up. Targeted drugs have a large market space and tend to be used in combination with cytotoxic drugs. Compared with import enterprises, state-owned enterprises need to improve their ability to make innovations. enterprises, state-owned enterprises need to improve their ability to make innovations.

    Objective To conduct bibliometric analysis of pharmacoeconomic evaluation of anticancer agents. Methods Related literature and information were retrieved from Web of Science from inception to December 31, 2020. VosViewer software and Excel were used to analyze the trends relating to the volume of articles published in this field, country/regional contribution, contribution of research institutions, contribution of scholars, funds and hotspots of research. Results A total of 550 articles were included in the analysis. The overall number of published papers in this field continued upwards, especially in the past five years. The number of papers on anti-tumor pharmacoeconomics published in China was gradually leading the world. Drugs for non-small cell lung cancer (NSCLC) and new anticancer agents have become priorities of research in recent years. Conclusion The research on pharmacoeconomic evaluation of anticancer agents is fast-developing globally with the change of disease spectra and the acceleration of new drug research. With the implementation of policies on negotiations about national medical insurance drugs, the related research in China will gain momentum.

    Amounts of Tyrosine Kinase Inhibitors Used in Hospitals in Nanjing from 2017 to 2019

    Objective To analyze the changes of usage of tyrosine kinase inhibitors in hospitals in Nanjing between 2017 and 2019. Methods The types, volume of sales, defined daily dose system (DDDs), and defined daily dose cost (DDDc) of tyrosine kinase inhibitors in 36 hospitals in Nanjing between 2017 and 2019 were analyzed statistically. Results The sales volume and DDDs of tyrosine kinase inhibitors in 36 hospitals in Nanjing increased at an average annual rate of 102.42% and 111.64% respectively between 2017 and 2019. Gefitinib remained among the top three in terms of sales amount and DDDs for three consecutive years. Osimertinib and anlotinib showed the fastest growth in sales amount and DDDs. Most of the tyrosine kinase inhibitor drugs trended up in DDDs except imatinib, the DDDs value of which decreased year by year. In addition, the sales amount and order ratio of dasatinib and nilotinib decreased simultaneously. Conclusion Tyrosine kinase inhibitors are widely used in hospitals in Nanjing, with a rapid growth rate in consumption and sales amount despite the decline in the DDDc of most of the drugs.

    Objective To study the effect of Poria on behavior and intestinal microflora of chronic unpredictable depression stress model rats. Methods A chronic unpredictable mild stress (CUMS) model of rats was used to conduct a sugar water preference experiment to evaluate the lack of pleasure in rats administered with Poria cocos, and an open field experiment was used to analyze the behavior of rats. 16 rRNA gene sequencing method for intestinal flora species difference analysis. Results Compared with the normal group, the model group's body weight and sugar water consumption rate decreased significantly, autonomous activity was significantly weakened, the intestinal flora species richness and species diversity decreased, and the levels of Bacteroides and Proteobacteria at the portal level increased High, the level of thick-walled phylum decreased, and the level of genus lactic acid bacteria decreased; after the rats were given Poria powder, compared with the model group, the body weight and sugar water consumption of Poria powder group increased significantly (P<0.05), autonomous Significantly increased activity (increased moving distance, increased number of entry into the center, increased number of verticals) and the difference was extremely significant (P<0.01), intestinal flora species richness and species diversity increased, Bacteroidetes and Proteobacteria at the portal level The level of phylum decreased, the level of thick-walled phylum increased; the level of lactic acid bacteria at the genus level increased.     Objective To explore the mechanism by which emodin inhibits macrophage activation, explain its role in the occurrence of melanosis coli, and find out about the potential risk of emodin induced melanosis coli. Methods Bioinformatics technology was used to screen and analyze the gene chips GSE 73309 and GSE 73310 related to inhibition of macrophage activation by emodin in GEO database. Gene Ontology(GO) annotation, Kyoto Encyclopedia of Genes Genomes (KEGG) pathway analysis, and Protein-protein interaction network analysis were performed on differentially expressed genes. By retrieving literature on clinical applications and basic research of emodin, the biological basis of rhubarb-induced melanosis coli was explored, and the results of biological information analysis were mutually confirmed. Results The biological pathways through which emodin inhibited macrophage activation were mainly manifested in inflammatory responses and cancer pathways related to tumor necrosis factors (TNF) and hypoxia inducible factors (HIF). Conclusion The mechanism by which emodin induces melanosis coli is closely related to the inflammatory reaction induced by emodin. In addition, emodin can potentially induce breast cancer, ovarian cancer and lung airway inflammation.

    Objective To study the biological mechanism of luteolin protecting against cerebral ischemia by network pharmacology. Methods The target of luteolin was screened by TCMSP database, target genes of CI diseases were screened by Gene Cards database, Luteolin-target network was constructed by Cytoscape software, protein-protein interaction(PPI) network was analyzed using STRING database, GO enrichment analysis and KEGG signal pathway analysis were performed on the relevant targets. Results 50 targets of luteolin on CI was acquired, and key targets protecting against CI by regulating vascular proliferation and generation, inflammatory response, apoptosis and other functions. Conclusion Luteolin plays an important role in cerebral ischemia by regulating oxidative stress, proliferation and regeneration of vascular smooth muscle, apoptotic signaling pathway, PI3K Akt signaling pathway and IL-17 signaling pathway

    Objective To report the progress in the implementation of MedDRA during the post-marketing monitoring of drugs in China in order to provide reference for complete implementation of MedDRA. Methods MedDRA was outlined. The problems with and challenges to the implementation and transformation of MedDRA were discussed. Ways to implement MedDRA were recommended. Results and Conclusion MedDRA is a medical term widely used in the field of global drug regulation. The implementation of MedDRA is conducive to the sharing and exchange of post-marketing monitoring data on drugs, but has to be steadily advanced in light of the actual situation in China.

    Objective To analyze the levels of reporting and monitoring of adverse drug reactions of essential medicines in recent years in China so as to provide reference for clinical safe use. Methods The Annual Reports on National ADR Monitoring issued by the National Medical Products Administration between 2015 and 2019 were collected while those on ADR/ADE involving national essential medicines were summarized and analyzed. Results From 2015 to 2019, reports of ADR/ADE caused by essential medicines in China kept increasing, which was consistent with the overall trend of reports in China. Conclusion With the improvement of regulations on essential medicines in China, ADR related to essential medicines deserve attention in order to promote the safe clinical use.

    Information Exchange Mechanism of Cosmetic Safety Risks in China

    Objective To analyze the information exchange mechanism for cosmetic safety risks in China in order to provide reference for upgrading the system of information exchange related to safety risks of cosmetics in China. Methods The way in which information was exchanged by Cosmetics Vigilance Briefing(2019)was analyzed, while the means by which information about risks of cosmetics was exchanged were categorized before recommendations were made. Results A variety of channels for release of information on safety risks of cosmetics were opened to the public and achieved good social benefits, but the mechanism of exchange needed to be improved. Conclusion More mechanisms for such exchange of information with the public should be made accessible.

    Applicability of HLA-B*5801 Allele Detection in Allopurinol Irritated Severe Allergic Reactions

    Objective To analyze the detection results of HLA-B*5801 and clinical applicability of allopurinol in our hospital in order to provide information for preventing allopurinol-caused adverse reactions. Methods The clinical information about patients who had their HLA-B*5801 detected between January 1, and December 31, 2017 in our hospital was retrospectively analyzed. The applicability of gene detection results was assessed. Results A total of 55 patients who had received gene detection were collected, including 39 from the Department of Nephrology, 13 from the Department of Dermatology, and 3 from the Department of Endocrinology. There were 17 (30.91%) HLA-B*5801 allele positive cases,38 (69.09%) negative cases. Thirteen patients suffered from cutaneous adverse reactions after allopurinol treatment, eleven of whom were HLA-B*5801 allele positive and 2 were negative. Among these patients, 30 (54.55%) had had their HLA-B*5801 gene tested before allopurinol therapy according to the guidelines. Conclusion The proportion of patients who have their HLA-B*5801 gene detected before allopurinol therapy in our hospital is relatively low. The pharmaceutical department should strengthen the supervision of clinical use of allopurinol. It is recommended that this drug be used according to the results of gene test to minimize the risk of adverse reactions.

    Adverse Reactions of Ultra-low Dose Decitabine in Elderly Patients with Intermediate- to high- risk Myelodysplastic Syndromes

    Objective To investigate the clinical efficacy and adverse reactions of ultra-low dose decitabine in elderly patients with intermediate-to high- risk myelodysplastic syndromes (MDS). Methods The clinical data on nineteen patients with intermediate-to high-risk MDS treated with ultra-low dose decitabine between July 1, 2014 and December 31, 2018 was retrospectively analyzed. The effect of age(aged ≥ 65 / <65) on curative effects and adverse reactions was studied. Results Nineteen patients received a total of eighty-one cycles of ultra-low dose decitabine. Eight (aged≥65) of these patients received a total of 26 cycles of treatment. Alteration of the natural course of disease response, complete remission(CR), and hematology improvement-erythrocyte reaction(HI-E) were much less likely among elderly patients than among those under 65 (P values were 0.024, 0.045, 0.024, respectively). There was no significant difference in other outcomes or in the incidence of post-treatment infection or grade III-IV /IV myelosuppression. With the increase of chemotherapy cycles, the incidence of myelosuppression did not increase significantly. Conclusion For elderly patients with intermediate-to high-risk MDS, the treatment with ultra-low dose decitabine can obtain better clinical efficacy without significant increase in adverse reactions.

    Comparison Analysis of Adverse Drug Reactions of Rivaroxaban and Apixaban Based on OpenFDA

    Objective To search for reports of adverse reactions caused by oral anticoagulant drugs-rivaroxaban and apixaban by using openFDA, and compare the adverse reactions of the two drugs in order to provide reference for clinical rational drug use. Methods Reports of adverse reactions caused by rivaroxaban and apixaban that were submitted between January 1, 2004 and August 12, 2019 to the openFDA database were searched for using the interactive chart section access API for Adverse Drug Reaction Endpoints. Relevant data and charts were retrieved and analyzed. Results There were 6 676 and 10 513 ADR reports of rivaroxaban and apixaban respectively, which were mostly reported by doctors. The adverse reactions occurred in the United States, and the affected population ranged from 65 to 82 years old. Rivaroxaban and apixaban could be used for the prevention and treatment of deep vein thrombosis and pulmonary embolism, but the main adverse reaction was bleeding. Conclusion Based on the openFDA large databases, the ADR related information of the two drugs can be analyzed, which provides data for effective drug monitoring and immediate treatment of adverse reactions, but the data has some flaws and needs to be improved.

    Analysis of 5 066 Cases of Drug-induced Liver Injury from 570 Medical Institutions in Shandong

    Objective To investigate the incidence and characteristics of drug-induced liver injury (DILI) in Shandong province over the past five years, and provide reference for pharmacovigilance and clinical safety of medication. Methods From the database of Shandong ADR Monitoring Center, cases of DILI reported between January 1, 2013 and July 31, 2018 were retrieved and screened. Data on the gender, age, ethnicity, disease history, varieties of drugs, drug categories, routes of administration, latency, primary diseases and disease outcomes of patients was collected and analyzed. Results Among the 5 530 reported cases, 5 066 cases met DILI diagnostic criteria, who were outpatients, emergency patients or inpatients from 570 medical institutions. The ratio of males to females was about 1.42∶1, and the incidence was the highest (1 991 cases, 39.34%) among patients aged 40 to 59. There were 17 categories and 789 kinds of drugs involved, among which anti-infective drugs (including anti-tuberculosis drugs) accounted for the largest proportion (2 537 cases, 34.81%). Oral administration was the most common (3 411cases, 67.33%). Tuberculosis was the dominating primary disease (933 cases, 18.42%). The median latency was 52.09 (35.12, 104.05) d, while the most common latency was 7~14 d(1 682 cases, 33.23%). The median indexes of main liver function were ALT 130.00(90.00, 215.00)U/L, AST 90.00(65.00, 150.00)U/L,TB 52.09(35.13, 104.05)μmol/L, DB36.07(16.70, 80.00) μmol/L. Most patients recovered or improved after treatment (4 298 cases, 84.84%). Conclusion Clinical medical staff need to improve their comprehensive understanding of DILI and strengthen the monitoring of high-risk groups and high-risk drugs.

    Objective To analyze such severe adverse reactions as acute drug-induced liver injury caused by vancomycin hydrochloride. Methods One case of liver injury after craniocerebral surgery was investigated. The cause of liver injury was identified by analyzing the diagnosis and treatment of the patient, and the suspected drug was evaluated by the RUCAM scale. Results The significantly elevated transaminase level in this patient might have been related to vancomycin, which was considered to have caused the acute drug-induced liver injury. Conclusion When using vancomycin, physicians and pharmacists should pay attention to the liver function and renal function of patients. In case of liver injury, they should be alert to the serious adverse reactions caused by vancomycin.

    Objective To analyse a case of vein thrombosis induced by dezocin injection and related literatures, in order to provide references for clinical rational use of dezocin injection. Methods Based on the rare case report, the domestic and foreign reports of adverse reactions caused by dezocin before December 2019 were searched and analyzed. Results The mechanism of thromboembolism induced by dezocine in this case might be injury of endothelial cells, the change of blood components and hemodynamic abnormality. Serious adverse drug reactions reported less, which included a small amount of anaphylactic shock reports. Conclusion For the rational use of dezocine injection, the manufacturer should further monitor its risks, and revise and improve the relevant contents of the drug insert to ensure the safety of clinical medication.

    Nucleos(t)ide analogues (NAs)-resistant hepatitis B is caused by gene mutations in the reverse transcriptase region of hepatitis B virus (HBV) after long-term use of NAs. NAs resistance could lead to treatment failure, and it is also one of the important reasons for liver disease progression. Clarifying the mechanism of HBV NAs resistance is of great help to strategy selection and prognosis treatment; combing the mutation site of HBV NAs resistance will provide the basis for further exploring the risk signals of drug-resistant hepatitis B and clinical precise prevention and control. This review analyzed the molecular mechanism and mutation site of NAs-resistant hepatitis B, hoping to provide a reference for the prevention and treatment of NAs-resistant hepatitis B.
