注射用还原型谷胱甘肽致过敏性休克文献分析

Literature Analysis of Anaphylactic Shock Induced by Reduced Glutathione for Injection

摘要: 目的 分析注射用还原型谷胱甘肽致过敏性休克的文献,探讨其致过敏性休克的规律和原因,为临床合理用药提供参考。方法 检索中国知网、万方数据、维普网和Pubmed数据库,检索日期1979年1月1日至2019年9月30日,收集国内外注射用还原型谷胱甘肽导致的过敏性休克个案报道信息,统计分析病例报告中患者年龄、性别、原患疾病、药物食物过敏史、给药途径、给药剂量、用药时间、合并用药、临床表现和转归等情况。结果 检索到27篇注射用还原型谷胱甘肽导致的过敏性休克病例报告,共计30例。其中20~50岁人群(58.6%)为过敏性休克发生的高危人群,男性(58.6%)高于女性(41.4%);以速发型过敏性休克为主,即静脉滴注开始后 30 min内发生者占比最高93.9%;以呼吸系统(胸闷、呼吸困难、气喘)和循环系统(血压骤降、口唇发绀)为首发症状。结论 注射用还原型谷胱甘肽可引起过敏性休克,严重者可致死亡。患者用药后 30 min内注意观察临床症状,避免严重事件发生。

Abstract: Objective To explore the clinical characteristics and regularity of anaphylactic shock by caused by reduced glutathione for injection in order to provide data for clinical rational drug use. Methods Related literature published between January 1979 and September 2019 was retrieved from CNKI, Wanfang databases, VIP and Pubmed. The age, gender, daily dose, time for medication of anaphylactic shock, combined medication, preexisting diseases, clinical manifestations as well as prognosis were analyzed. Results A total of 27 case reports related to anaphylactic shock and involving 30 patients were collected. Patients with immediate anaphylactic shock (within 30 minutes of medication ) accounted for 93.9% of the total. The initial symptoms of anaphylactic shock mainly involved the respiratory system (chest tightness, dyspnea and asthma) and the circulatory system (sudden drop of blood pressure, cyanosis of the mouth and lips). Conclusion Anaphylactic shock caused by reduced glutathione for injection in severe cases can lead to fatal events, especially within 30 minutes. The symptoms of patients should be closely monitored to prevent fatal events.
