某院2015-2017年382例硫酸鱼精蛋白注射液用药及其药品不良反应分析

Analysis on 382 Cases of Medication and Drug Adverse Reactions of Protamine Sulfate Injection in a Hospital from 2015 to 2017

摘要： 目的 分析硫酸鱼精蛋白注射液临床用药情况及药品不良反应,为临床合理用药提供参考。方法 应用中国医院药物警戒系统（CHPS）检索病历,回顾性分析某院2015年1月至2017年2月382例使用硫酸鱼精蛋白注射液的用药情况及其药品不良反应/事件（ADR/ADE）发生特点。结果 除1例患者单次用量300 mg外,其他用药无明显不合理。ADR 8例,主要为低血压,其中严重ADR 2例,分别为过敏性休克和心肌抑制。更换使用新批号硫酸鱼精蛋白注射液并减少其用量,对预防ADR起到积极作用。结论 某院硫酸鱼精蛋白注射液用药基本合理,ADR主要为血压下降,医务人员应加强记载用药详细情况并主动上报ADR,以便评价用药风险,促进临床合理用药。

Abstract: Objective To analyze the rational uses and adverse reactions of protamine sulfate injection in the clinic, and to provide references for rational drug use. Methods We conducted a retrospective review of 382 inpatients with application of protamine sulfate injection from January 2015 to February 2017 in a hospital, which were collected by China Hospital Pharmcovigilance System (CHPS), and analyzed the characters of the adverse drug reaction/event (ADR/ADE). Results There weren't obviously irrational medication orders except one case that the patient was treated with a single dose of 300 mg protamine sulfate. 8 cases of ADR were obtained, and they were mainly hypotension. And 2 cases of severe ADR were anaphylactic shock and myocardium inhibition. A new batch of the protamine sulfate and dose reduction had positive effects on preventing adverse reactions. Conclusion The use of protamine sulfate injection in this hospital was basically rational, and the ADRs were mainly hypotension. In order to assess the risk of medication and promote clinical rational use of drugs, medical staff should strengthen to write treatment details and actively report ADRs.
