摘要/Abstract

摘要： 目的探讨肾移植合并乙肝病毒（hepatitis B virus, HBV）感染患者的药学监护措施。方法临床药师对1例二次肾移植合并HBV感染的患者制定药学监护计划,对其诊疗过程中出现的异常指标进行监护分析,并提出干预建议,为患者提供个体化药学服务。结果患者诊疗过程中出现的尿蛋白及尿微量白蛋白转阴、病毒相关性肝损害好转,他克莫司血药浓度控制达标。结论临床药师利用药学专业知识对患者进行个体化治疗监护,对肾移植合并HBV感染患者进行安全合理用药非常有必要。

Abstract: Objective To explore pharmaceutical care methods for patients with hepatitis B virus (HBV) infection after second renal transplantation. Methods Clinical pharmacist provided individualized pharmaceutical care for a patient with HBV infection after second renal transplantation, analyzed the abnormal indexes during treatment, and offered recommendations about pharmacy. Results Urine protein and urine microalbumin disappeared, hepatitis B virus-related liver damage was improved, and the tacrolimus concentration was controlled to reach the standard. Conclusion Clinical pharmacists need to offer individualized pharmaceutical care for patients with hepatitis B virus infection after renal transplantation.
