硫唑嘌呤致血液学毒性危险因素的初步研究

Tentative Study of Risk Factors of Hematological Toxicity Induced by Azathioprine

摘要: 目的 应用Logistic回归模型和ROC曲线分析硫唑嘌呤所致血液学毒性发生的危险因素,以减少用药风险。方法 从医院HIS信息系统收集患者相关信息,采用SPSS 22.0软件进行统计分析。结果 共纳入70例风湿病患者,28例患者出现血液学毒性。Logistic回归模型单因素分析结果显示,BMI、NRS2002评分、前白蛋白、NUDT15基因多态性4个因素的差异具有显著性。进一步多因素Logistic回归模型分析表明,NRS2002评分是发生血液学毒性的独立危险因素。以NRS2002构建ROC曲线,NRS2002≥3分时,患者更易发生血液学毒性。结论 NRS2002评分可预测服用硫唑嘌呤的患者发生血液学毒性的风险,给予饮食指导,提高用药安全。

Abstract: Objective To explore the risk factors of hematological toxicity induced by azathioprine drugs via a logistic model and ROC curve. Methods Related information about patients was collected from the HIS system of our hospital, and SPSS 22.0 software was used for statistical analysis. Results A total of 70 patients with rheumatism were included, 28 of whom suffered from hematological toxicity. Logistic analysis showed statistically significant differences in BMI, NRS 2002, prealbumin, and NUDT15 polymorphism. Further multivariate logistic model analysis showed that the NRS2002 score was an independent risk factor for hematological toxicity. An ROC curve was constructed with NRS2002, and patients were more likely to suffer from hematological toxicity when NRS2002 exceeded 3 points. Conclusion The risk of haematological toxicity in azathioprine-treated patients can be predicted by the NRS2002 score so that drug safety can be ensured by providing dietary guidance.
